Each year, UFCW 2013 Health & Welfare Fund participants receive an IRS Form 1095-B. This is a tax document that shows proof of your health coverage for the previous calendar year.
This form lists the months you (and any covered dependents) had minimum essential coverage under your health plan, as required by the Affordable Care Act (ACA). While you don’t need to file the 1095-B with your federal tax return, it’s important to keep it for your records in case you need to verify your coverage with the IRS.
